Supplemental Benefits Provided by the
Civil Service Employee Association
Benefit Trust Fund

 

Overview

The CSEA Benefit Trust Fund provides supplemental benefits to full-time Civil Service employees (i.e., employees appointed to work more than 25 hours per week).  These benefits include:

  • Dental coverage
  • Vision coverage
  • Group life insurance
  • Accidental death/dismemberment insurance 

The following supplemental benefits are provided at no cost to the employee:

  • Vision coverage
  • Group life insurance
  • Accidental death/dismemberment insurance

Dental Coverage

 

Dental Coverage is provided to active members who have been employed by the District for at least 6 months at an annual employee premium of $50 for single coverage or $200 for family coverage.  The total benefit for an active unit member's family is limited to $700 in the first year of coverage.  Participating members are entitled to full benefits after the first year of coverage.

This benefit allows members to visit the dentist of their choice and covers 100% of the scheduled allowance for preventive treatment, emergency treatment, diagnostic services, anesthetics, injectible antibiotics, extractions and oral surgery, fillings, crown/onlays and inlays, periodontics or treatment of gum disease, root canal therapy, orthodontics, dental prosthetics, and implants.  Note: certain procedures have benefit limits and time-related restrictions.  Members are advised to consult the complete benefit details to review applicable limits and restrictions prior to seeking dental services.

Vision Coverage

Vision coverage is provided through Davis Vision™ at no annual cost to active members who have been employed by the District for at least 6 months. 

This benefit allows members to receive one eye examination per year and covers the cost of one pair of corrective glasses per year or the cost of corrective contact lenses, up to the scheduled allowance for these services.  For an overview of the benefit details, click on the Vision Care Plan Benefit DescriptionMembers are advised to consult the complete benefit details to review applicable limits and restrictions prior to seeking vision services.

To view a list of approved service providers offered by Davis Vision™, log on to www.davisvision.com or call 1-800-999-5431.  If you use a service provider outside of the Davis Vision Network, you must complete and submit the Direct Reimbursement Claim Form.

Life Insurance Coverage

Life insurance coverage is provided through Unum Provident™ to eligible unit members who work at least 30 hours per week. 

This benefit provides members with $15,000 of group life insurance coverage.  Eligible members may elect to purchase additional life insurance through Unum Provident™. To enroll in life insurance coverage, eligible members must complete the Group Insurance Enrollment Form and submit it to the Office of Human Resources.

Accidental Death & Dismemberment Insurance Coverage

Eligible employees are are entitled to $15,000 of insurance coverage towards basic accidental death, dismemberment and loss of sight. To enroll in this benefit, eligible employees must complete the Accidental Death and Dismemberment Enrolment Form.
