Sports physicals will be held on the following days;

Any 7/8 grade student interested in participating in a sport must have:
 
  - a current physical exam on file in the nurse's office
 
 
 
These forms should be sent to the nurse ASAP.
 
In addition, if your child has an Epi-pen or inhaler, a written physician's order and 2 inhalers should be provided to the nurse.
 
You must include a Medical Authorization Form
