Workers Compensation

All work-related injuries must be reported to your supervisor immediately and an HCSD Incident Report must be completed by your supervisor and the school nurse as soon as possible, but not more than twenty-four (24) hours after the injury.  In order for the District to open a claim on your behalf, an incident report is needed as soon as possible, and late filings of incident reports may subject the District to a fine by the Workers Compensation Board.

You will need to provide the doctor or hospital with a workers compensation claim number.  If you don't have a claim number, provide the carrier code above.  You are not required to pay for any treatments, co-payments, deductibles or make any partial payments for treatment due to work-related injuries.
